who are already in a time of crisis.
Care Connect's approach is a standardized, scalable model that closes gaps
and breaks down silos, allowing those in need to more easily access available
resources in the community. More than merely providing information and
referrals, clients also have a navigator throughout the process who is
invested in their success.
1. Assess
We review the local Community Health Needs Assessment and learn more about
existing resources to better understand the unique needs of the area.
2. Develop
Care Connect develops a strategic plan and corresponding timeline to implement
the model.
3. Implement
Using shared leadership and an individualized toolkit, we work hand-in-hand
with the community to execute the plan.
4. Analyze
Care Connect develops reporting infrastructure to collect and analyze success
metrics unique to the community. This allows you to visualize the impact
of the program’s implementation.
5. Innovate
The model is flexible enough to grow as new community needs are identified
and the addition of resources become available.
