The Care Connect Process This is Alice

Alice is 30 years old and has two children. Alice needs help with her past due light bill and needs food for her family. But, Alice is confused by all of the programs and resources available to her to help with her situation. Alice is unsure of where to begin.

  • Step 1
    Alice goes to the Care Connect+ website and fills out a "Get Connected" form in search of help.
  • Step 2
    Our Intake Specialist receives Alice’s form and reaches out to her to confirm the details on her form.
  • Step 3
    Alice is assigned to a Community Health Associate who reaches out to her. Alice and the Community Health Associate work together to navigate the programs and resources that she is eligible for.
  • Step 4
    Over the next two weeks, Alice is connected to local food resources and her Community Health Associate helps Alice sign up for a program that can help her get caught up with her light bill. Alice is able to successfully obtain a mobile phone and transportation to and from her job.
  • Step 5
    Alice leaves Care Connect+ equipped with the tools she needs to succeed and fulfill her family’s needs!
